Community Support
Services offers short and long-term community support (also
known as Case Management) for adults with a serious mental
illness. Community Support Workers are available to assist
individuals to set goals in a variety of areas, including
those on Community Treatment Orders, to maintain stability
in the community.
Services Include
- Assessment and individual goal-setting
- Information on available resources and linkage with
programs and services
- Advocacy
- Personal support counselling
- Culturally appropriate services (interpreter
provided)
- Life skills teaching
- Education about mental illness
- Symptom Management
- Monitoring of medications and side effects
- Assistance finding housing
- Support is available to avoid hospitalization,
homelessness and incarceration
- Peer Support
