Client Population
- adults 16 years of age and older with a serious mental illness and their families
- service provided in English - staff also speak Ukrainian, Italian, French, Dutch, Bengali & Hindi
 - if other languages needed, interpreter contacted through YMCA and paid for through our Agency
 - we serve urban Aboriginal clients and Aboriginal clients living on Six Nations at the client's request
 - services provided across various functional centres to achieve the following client outcomes:
  - improve daily functioning and quality of life
- reduce use of emergency departments and need for hospitalization
- maintain stable affordable housing
- find and maintain education, volunteer placements and paid employment
- cope better with the challenges of supporting a family member with a mental illness
- have meaningful social and recreational activities in their lives
- acquire skills in various areas of living
- reduce time in custody and conflict with the law
Geography Served
 - catchment area is primarily considered to be the boundaries of Brant County; however clients who choose to
   access services in Brantford bur reside outside these boundaries, are accepted if they can get to our office.
 - our main office is located at 44 King Street, Brantford, Ontario, open 8:30 a.m. to 4:30 p.m. Monday to Friday.
 - we have two supportive housing sites at Phoenix Place Apartments, 175 Dalhousie Street, Brantford, where
   our Property Manager is located Monday to Friday (24 hrs/wk) and at 20 Lyons Avenue, Brantford.
 - much of our community support work is provided in clients' homes Monday to Friday 8:30 to 4:30 unless
    times are arranged outside of these hours on a case by case basis depending need and worker avilability.
